Cancer Incidence In Dakota And Washington Counties
In response to concerns about cancer and the presence of Per- and Polyfluoroalkyl substances ) in drinking water supplies in the eastern Metro area communities, the Minnesota Department of Health has twice examined cancer incidence in Dakota County and Washington County.
MDH released the first report in 2007, which was followed by an updated report in 2015. While community cancer rates have a high degree of statistical uncertainty and must be interpreted cautiously, these reports can be useful to address public concerns over cancer rates in a county or a community by providing a complete and accurate profile of cancer occurrence.
Recent concerns arising from media coverage of the State of Minnesota lawsuit against 3M, prompted MDH to re-examine and expand on the two previous reports of cancer incidence in Washington and Dakota Counties, and eight communities within those counties.

Breast Cancer Incidence Rates Over Time
In the 1980s and 1990s, the rate of breast cancer incidence rose, largely due to increased breast cancer screening with mammography .
The rate of breast cancer incidence declined in the early 2000s . This decline appears to be related to the drop in menopausal hormone therapy use after it was shown to increase the risk of breast cancer .
Mammography screening rates also fell somewhat in the early 2000s. However, studies show the decline in the rate of breast cancer incidence during this time was not likely due to the decline in screening rates .
Since about 2005, the overall breast cancer incidence rate has increased slightly . This may be due, in part, to an increase in body weight and a decline in the number of births among women in the U.S. over time .
Trends in incidence rates of breast cancer may be different among some groups of women.

Screening And Early Detection Of Breast Cancer
When screening catches cancers early, theyre easier to treat and cure. The low mortality rate of breast cancer has a lot to do with how early cancer is detectedoften before the person can feel a lump.
The usual way to screen for breast cancer is a mammogram. A mammogram machine takes a picture of the breast tissue using X-rays.
The American Cancer Society recommends that women with average risk who are 45 to 54 should get a yearly mammogram to catch breast cancer early. At age 55 and older, they may switch to getting a mammogram every two years or continue with yearly mammograms.
For every 1,000 screenings performed, mammograms detect five breast cancers. Without mammograms, many of these cancers would progress before being found.
A recent study in the ACS journal Cancer showed that women who got mammograms had a 41% reduced risk of dying from breast cancer within 10 years and a 25% reduced chance of being diagnosed with advanced breast cancer.
People who are transgender should discuss their risks with their healthcare provider to determine whether they may benefit from screening for breast cancer.

Incidence Rates And The Number Of New Cases
To know whether or not breast cancer rates are changing over time, you have to compare rates, rather than the number of new cases.
For example, lets compare the number of new cases of breast cancer in U.S. in 2009 to the number of new cases in 2016. In 2009, there were an estimated 192,370 new cases of breast cancer in U.S. women . In 2016, there were an estimated 246,660 new cases .
Although more breast cancer cases occurred in 2016 than in 2009, this doesnt mean the rate of breast cancer increased over this time period.
We expect the number of cases to increase over time because the population of the U.S. increases over time . The more people there are, the more cancers there will be.
Our population is also living longer, so there are more people who are older . Since age increases the risk of breast cancer, we expect to have more breast cancers over time.
To know if breast cancer rates are changing over time, we look at incidence rates, rather than the number of new cases. The incidence rate shows the number of breast cancer cases in a set population size over a period of time. Its usually written as the number of cases in a population of 100,000 people per year.
The breast cancer incidence rate among women in 2009 was 125 and the estimated breast cancer incidence rate in 2016 was also 125 . This means there were 125 breast cancer new cases per 100,000 women in the U.S. in both 2009 and 2016.

Genetics And Family History
- Around 15-20% of men and women with breast cancer will have a family history of the disease
- Inherited mutations in BRCA1 and BRCA2 genes account for about 4-6% of all breast cancer cases in women and around 11-12% of cases in men

How Many People Survive Breast Cancer
- Almost nine in ten of women survive breast cancer for five years or more.

Efforts To Reduce Or Eliminate Exposures
MDH is responsible for ensuring safe drinking water for all Minnesotans. One way MDH does this is through regular testing of public water supplies for contaminants. MDH also works with the Minnesota Pollution Control Agency to investigate situations where groundwater contaminants may affect private wells.
From 2008 to 2014, MDH led a biomonitoring study that tracked blood levels of PFCs in people who live in the East Metro.
The biomonitoring study results show that:
- PFC blood levels are going down in long-term residents who were exposed to PFCs in drinking water before the 2006 public health intervention. That intervention, including installing filtration systems for polluted public and private wells, reduced PFCs in drinking water to below health-based limits.

PFC levels in newer residents â who moved to Oakdale after the intervention â are similar to levels seen elsewhere in the U.S.
This MDH analysis found that the North Minneapolis neighborhoods included in ZIP codes 55411 and 55412 had a greater overall cancer incidence when compared to the metro area but did not have a significantly different cancer burden compared to the broader African American community in the metro area. African Americans comprised 8.6 percent of the Twin Cities Metro in 2010 but made up nearly 46 percent of the population in North Minneapolis.

Relative survival is an estimate of the percentage of patients who would be expected to survive the effects of their cancer. It excludes the risk of dying from other causes. Because survival statistics are based on large groups of people, they cannot be used to predict exactly what will happen to an individual patient. No two patients are entirely alike, and treatment and responses to treatment can vary greatly.

What Is Survival Rate
The survival rate is the percentage of people who survive a disease such as cancer for a specified amount of time after diagnosis. But it is presented in many different ways.
Between 2015 and 2019, about 1 in 5,000 women in the U.S. died of breast cancer. Mortality rates for breast cancer have been declining steadily since 1990.
Before 1990, mortality rates for breast cancer in women hovered between 30 to 35 women dying from breast cancer for every 100,000 women in the U.S. population. In 2019 this number dropped below 20 for the first time.
Between 1989 and 2017, breast cancer death dropped by 40%, and 375,900 deaths were avoided during those 28 years.

The stages of breast cancer are based on the following factors:
- whether the lymph nodes contain cancer cells
- whether the cancer has metastasized, meaning its spread to other, more distant parts of the body
Since 2018, the following factors have also been used to determine breast cancer stage:
- whether the cancer cells have hormone receptors and need estrogen or progesterone to grow
- whether the cancer cells have the HER2 protein that helps them grow
- tumor grade, meaning how aggressive the cells look under the microscope
Lifetime Risk Of Breast Cancer Worldwide
Women who live in developed countries tend to have a higher lifetime risk of breast cancer than women who live in developing countries .
Although we dont know all the reasons for these differences, lifestyle and reproductive factors likely play a large role .
Low screening mammography rates and incomplete reporting can make rates of breast cancer in developing countries look lower than they truly are and may also explain some of these differences.
