How To Report Alternative Testing
For those with a breast cancer diagnosis or for those at high risk of breast cancer, magnetic resonance imaging may be ordered. An MRI can provide clearer 3D images of the breast, which are then used to either assess the extent of breast cancer or to screen for breast cancer in women thought to have a high risk of the disease. As shown in Table C, codes 77046 and 77047 are reported for breast MRI without contrast. MRI with computer-aided detection can help radiologists identify abnormalities on breast MRI and is reported with codes 77048 and 77049, also shown in Table C.

Contrast-enhanced digital mammography may be also be ordered. A CEDM is a mammogram that uses iodinated contrast dye. This dye makes it easier to find new blood vessels that develop when cancers grow. CEDMs find breast cancers that cant be seen on regular mammograms, especially in women with dense breasts. Less expensive than an MRI, CEDMs may also have lower false-positive rates than breast MRIs, resulting in increased diagnostic accuracy. The codes are shown in Table D.

Journalist Katie Couric revealed on her website today that she was diagnosed with breast cancer in June of this year on the eighth anniversary of her marriage to John Molner.
Couric, who called herself the screen queen for colon cancer, underwent a colonoscopy live on the Today show in 2000 after the death of her husband, Jay Monahan, from colon cancer in 1998.
In a post titled, Why Not Me?, Couric describes her journey from diagnosis after a late mammogram to a biopsy, lumpectomy and radiation.
She says she learned in June that she was overdue for a mammogram. Her last one was in December 2020.
At the mammogram, Couric said she asked a technician to film the procedure in a very PG kind of way.
This wasnt exactly new territory for me. You might remember I aired my colonoscopy on the TODAY show in 2000. After that segment, the number of people getting colonoscopies increased by 20 percent. If I had forgotten to schedule a mammogram, this might be a helpful reminder for other people, too.
The doctor also performed an ultrasound and told Couric she saw something she wanted to do a biopsy. The doctor texted the next day because she could not reach Couric on her cell phone.

Katie Couric said Wednesday that shed been diagnosed with breast cancer, and underwent surgery and radiation treatment this summer to treat the tumor.
Couric, who memorably was tested for colon cancer on the Today show in 2000, announced her diagnosis in an essay on her website, saying she hoped it would encourage other women to be tested.
Couric, 65, was diagnosed on the first day of summer and wrote that she had her final radiation treatment on Tuesday.
My left breast does feel like Ive been sunbathing topless, but other than that, Ive felt fine, she wrote.
Courics first husband, Jay Monahan, died of colon cancer in 1998 at age 41 and her sister Emily was 54 when she died of pancreatic cancer in 2001. Given her family history, Couric wrote, why would I be spared? My reaction went from why me? to why not me?
The former host of Today and the CBS Evening News said she recorded her mammogram and breast biopsy with the intention of sharing it with followers, but her doctor asked her to turn off the cameras when she told her that a biopsy was necessary.
A day later, Couric was called and told she had breast cancer and needed to make a plan.

Diagnosis Coding For Possible Malignancy
How would you feel if your primary care physician and your general surgeon told youand told your insurance company on a claim formthat you had breast cancer, when you didnt have breast cancer?
You actually had atypical ductal hyperplasia, not breast cancer. Not only have they told you, the claim has gone to your insurance company and the insurance company thinks you have breast cancer. And, it is listed in your electronic medical record on your problem list.
Anyone who works in healthcare knows that removing a diagnosis from a medical record at the physician office, at the hospital, and in the insurance companys records will be difficult.
Think about the repercussions for this patient if she later needs to buy life insurance, disability insurance, or a health insurance in a state that allows charing higher rates based on pre-existing conditions.
How did this happen?
Neither the primary care physician nor the surgeon followed ICD-10 coding rules and coded the abnormal sign/symptom.
Instead, they coded the suspected diagnosis.
The patient had a screening mammography done. The mammogram showed clustered calcifications in one of her breasts and a low suspicion for malignancy. Soon thereafter, she had a biopsy. The result of that biopsy was atypical ductile hyperplasia bordering on ductal carcinoma in situ.
The surgeon also assigned D05.12, carcinoma in situ. Two claims have so far been submitted to this patients insurance company stating that she has carcinoma in situ.
