Diagnosis: From Mammography To Biopsy

Optimal treatment of breast cancer varies from patient to patient. At the beginning of a therapy, its a must to first find answers to: how big is the tumor? Where exactly is it? What do its cells look like on the surface? At what speed does it grow? In order to answer these questions, doctors in Germany use different examination methods.

MammographyDoctors take X-rays in order to make small non-palpable nodules in the breast that visible. Thus, they can see whether epithelial cells which line the milk ducts of the breast have mutated.

UltrasoundIf the X-ray image shows any abnormalities, an ultrasonic examination can be done. The tissue, the fluids and the torso have different densities and are thus reflected differently by the ultrasonic waves. Bones are displayed in white, fluids in black and the density of tissue is displayed in greyscale. With an ultrasound device, the doctor can for example determine whether the nodule is a cyst filled with liquid or a tissue node.

BiopsyA biopsy provides definite clarity whether a tissue mutation in the breast is benign or malignant. Biopsies can usually be carried out with a cannula under local anesthesia. In rare cases, the doctors use a surgical biopsy.

Depending on the development of the disease, further procedures are applied in order to determine the degree of the disease, such as galactography , thermography or computer tomography .

Survivor Stories: Toll Of Cancer Costs

An increasing number of studies show how unaffordable medical bills can impact a persons health as much as the biological effects of cancer treatments. Researchers call this financial toxicity.

The high costs of cancer treatments add distress, hardships and burden to a patient and their family. Out-of-pocket costs not only deplete checking and saving accounts but also can dictate treatment decisions.

These stories showcase how cancer can alter a persons financial future.

Considering The Pink Fund

Breast cancer patients who need financial help during their treatment can apply to The Pink Fund. The fund has several criteria for qualification, including that applicants “must have an annual household income of 500% or less of the federal poverty level, based on last year’s federal tax return.” For a single person, that income would be $62,450, according to a chart on the fund’s website.

The Pink Fund provides 90 days of cost-of-living expenses with a cap of $3,000, based on qualification, available program funds and need.

Also, The Pink Fund only considers applicants undergoing “active treatment” who can prove a loss of work income. The fund defines active treatment as: “The period after a positive diagnosis of breast cancer has been made , and during which therapies are being administered, including surgical procedures to remove the cancer , chemotherapy or radiation.”

What To Ask About Health Insurance Coverage Of Your Treatment

Out-of-pocket costs are those you have to pay because your health insurance doesnt or after your insurance company has paid its portion. These costs can add up quickly and may make it hard for you to pay for other things you need. Youll want to be sure that your health insurance company pays or reimburses the bulk of your medical expenses. This means youll need to

    • Know the terms of your insurance policy.
    • Be aware of preferred or network doctors, hospitals, or clinics according to your policy.
    • Keep careful records of your health care costs

If any of your treatments might be done by out-of-network doctors or providers, find out about those costs from your insurance company too. Even when you know the terms of your policy, getting payments can mean re-submitting claims, appealing denials, and much more.

Usually, health care facilities and treatment centers have a financial department that handles health insurance concerns and problems. Ask your health care team if someone can help you with claims and understanding codes on the bills that are sent to the insurance company.

    • We demonstrate that the cost of early breast cancer care was mainly driven by tumour subtype. Clinicians and stakeholders must be aware of the weight that targeted systemic therapies may have in the overall cost of care and, given the recent approval of many drugs for the treatment of early breast cancer, that this predominance will probably increase in the future.

    What Is Breast Cancer Treatment

There are several ways to treat breast cancer, depending on the type and stage of the cancerous area. Briefly explain, the stages of cancer are typically labeled as 1 to 4 , and they describe the size of cancer and how far it’s metastatic. Stage I means the cancer is smaller and it is seen still only in one area. This stage is called early-stage cancer. Stage II and stage III mean the cancer is larger than stage I and has already started to be grown in nearby tissues or lymph nodes. And stage IV means cancer has spread to other parts of your body. Cancer in this last stage is called advanced cancer or metastatic cancer.

The main breast cancer treatment options include:

    • Surgery,
    • Hormonal therapy
    • Biological therapy.

You might have a combination of these treatments depending on your condition. So, your doctor will take many significant factors into account when choosing the best and the most suitable treatment for you. It should also be mentioned here there are many cancer researchers conducting various studies on these treatments and trying to increase the efficacy of them, as we see on the latest newson breast cancer. For example, according to a breast cancer research, reducing the interval between chemotherapy treatment cycles or administering chemotherapy medications sequentially at higher doses, rather than simultaneously at lower doses, increases treatment efficacy and decreases the risk of breast cancer recurrence and death, a study suggests.

    Costs For Patients From Abroad

What are the costs for Breast Cancer treatment procedures? – Dr. Nanda Rajaneesh

The treatment of breast cancer varies from patient to patient, so you can not say in advance what the costs are. It is possible that only a small tumor in a breast is removed and not even radiation is necessary after that. Then the expenditure remains comparatively small. If a larger operation is performed and the subsequent therapies are expensive, the costs increase.

As a rule, foreign patients pay the same prices as German patients. No matter if youre in the hospital or in a private practice. Because the billing depends on the statutory cost catalogs. When individual services are added, such as a chief physician treatment or a single room, the prices increase. Explicitly for foreign patients, there come costs for travel, translation and accommodation.

Suppose a patient, has a breast infested with a malignant tumor and a major operation is necessary. The patient has already passed the menopause, the estrogen production of the ovaries is low. Therefore, the ovaries do not need to be removed. The treatment of this patient would take an average of 5.6 days and costs 4,673 euros. It is billed according to the case flat rate J23Z, which is the most frequently charged case lump sum in breast cancer patients. However, only the costs incurred during the time in the hospital are recorded here. The follow-up and preliminary examinations are extra calculated and differ from case to case.

In Some Practices Financial Discussions Are Built In And More Need To Adopt This Process

Fischer said that he typically focuses on patient-centric aspects of breast cancer care with respect to diagnosis, treatment, and prognosis.

We will recommend specific treatment protocol, said Fischer. Then a nurse will have a separate appointment with the patient to review the protocol and specific supportive care aspects of the protocol. Then they segue into financial care.

Fischer said that once the protocol is submitted to the insurer, the practices financial counselors talk to patients regarding their projected responsibilities.

We have a whole financial team. Theyre far savvier than physicians when it comes to which programs benefit which patients. It has a lot to do with which treatments will be used, he said.

Weber said that there used to be a line in the sand between the financial people and the care being provided, but their relationship with one another has evolved.

The doctors were very driven to get patients into care as quickly as possible. They felt the process of obtaining authorization was slowing their ability to treat patients. We had to get our doctors to understand that concerns about the financial impact of this disease can outweigh actual treatment of the disease, she explained.

Weber said doctors have begun to tune in to this reality.

Were now working in partnership. The doctors really try to raise a flag if they see something we can do. Theyre not just tuned in to the physical piece, but the emotional piece .

    The Uninsured Or Underinsured

Many people struggle to pay rising health insurance premiums and copays, or they cant reach high deductibles before insurance coverage begins.

According to the Commonwealth Fund Biennial Health Insurance Survey, 23% of U.S. adults ages 19-64 were underinsured in 2018, meaning their out-of-pocket health care costs, excluding premiums, equaled 10% or more of their yearly income.

More than 44 million Americans remain underinsured because of high out-of-pocket costs and deductibles. Another 28 million were uninsured in 2017, according to the U.S. Census Bureau. The majority of the uninsured were men ages 19 to 64 with less than a high school education, lower incomes or both.

In 2017, uninsured nonelderly adults were more thantwice as likely as their insured counterparts to have had problems paying medical bills in the past 12 months.
