Regardless of whether or not you have implants, its vital to know your breasts. As daft as it may sound, knowing your breasts could save your life. By knowing whats normal for you, spotting anything irregular is far easier. Breast cancer is the most common cancer in the UK, so its important to look after your breasts by checking them regularly.
Its important for those that have chosen to have breast implants to be able to identify their implants and residual scar tissue when performing their frequent self-examinations. It is useful to have your surgeon to help show you the implants and its edges, so you know exactly what it is you are feeling.
There are a few things to consider when self-examining of your breasts. The first being that you should not examine your breasts whilst youre still within the healing stage, up to 6 weeks post-operation. If you are unsure we advise you speak to your surgeon to ask when they feel is appropriate for you to start examining yourself. You should also be aware not to examine when your breasts are tender and/ or swollen, which tends to be when you are menstruating. It is also worth noting that some implant placements can push out the breast tissue, which can help make the breast self-examination easier.

Get familiar with your new breasts
In about 90% of breast cancer cases, women find a breast lump themselves. So, check your breasts regularly. Its one of the best ways to detect breast cancer early.
If you have implants, pay extra attention to how your new breasts look and feel.
Itll take a while to get used to your new breasts because theyll have a different texture. Theyll also have new folds or dimples, Bevers says.
But after you get comfortable with your new breasts, you shouldnt have any problems noticing changes.
Women with implants can sometimes even notice changes more easily than women without implants, Bevers says. Implants tend to push the natural breast tissue closer to the surface of the breast, making a lump easier to feel.
Take extra screening steps
Most women, including those with breast implants, should start annual mammograms at age 40.
Worried that flattening your breasts during the exam will damage your implants? That rarely happens, Clemens says.
Why Are Implants Linked To Cancer
Experts arent entirely sure why breast implants make you more likely to get ALCL. What they do know is that 93% to 96% of the women who get ALCL have textured implants. That means their casing is bumpy instead of smooth.
Like smooth implants, textured implants are used for cosmetic and reconstructive breast surgery. Textured implants are less likely to move or cause your skin to ripple. But experts think their textured surface may trigger some womens immune systems to respond in a way that leads to cancer.

Will The Mammogram Exam Damage My Implant
If this question is on your mind, youre not alone. During a mammogram your breast is compressed. Ive had patients come here that are concerned youre going to pop my implant, says Dr. Walters. But we have special views where we can push the implant up out of the way and then we pull the breast in so were compressing the breast. There are other imaging options if the implant cant be moved enough to X-ray all of the breast tissue. Some patients may need an ultrasound or MRI, both of which can be used to screen for breast cancer.
Implant Rupture With Mammography Is Rare
Worried that your implant might burst under compression? Its not a common occurrence.
A 2004 study in the Journal of Womens Health examined problems with mammography for women with breast implants. When researchers reviewed adverse events reported to the U.S. Food and Drug Administration , they identified just 44 incidents involving breast implant rupture with mammography.
In a separate review of published studies, FDA researchers identified another 17 cases involving breast implant rupture during compression.
Yet there are almost 300,000 women who undergo breast augmentation each year, says the American Society of Plastic Surgeons.
In many rupture cases, Dr. Baker suspects the implant was already compromised and the compression just helped it along.

How Do I Spot The Symptoms
It is important to know the symptoms of breast implant associated cancer. The most common symptom is swelling of a breast caused by fluid build-up around the implant, but in some cases it may appear as a lump in the breast or armpit.
As this is a rare disease, changes in your breast are unlikely to be breast implant associated cancer. For example, swelling immediately after your breast implant surgery is normal. But you should raise all concerns with your surgeon, including the possibility of breast implant associated cancer. If the surgeon or clinic which performed your original implant operation is no longer available, you should see your GP for referral to another surgeon.
A Note About Fibrocystic Breast Tissue
Fibrocystic breasts are breasts that always feel dense and lumpy. This is a very common condition and doesnt necessarily mean that the breasts are cancerous. Once your doctor helps you establish which lumps are noncancerous, you can use these as benchmarks to observe any changes in the natural landscape of your breasts.
Since breast implants are placed under your breast tissue, you can still have fibrocystic breasts following an augmentation.

How To Perform A Breast Self

Should I Consider A 3d Mammogram Also Called Tomosynthesis Over A 2d Mammogram
While a 2D mammogram uses a single image to evaluate the breast, a 3D mammogram takes many pictures to show thin layers of the breast and produce a 3D image. These views can make it easier to spot abnormalities in the breast, especially for women with dense tissue.
The X-ray dose for 3D mammograms can be higher than a standard mammogram but it is still well within safety limits, Dr. Reddy said. At MD Anderson, we use a low dose technique for 3D mammograms which reduces the radiation dose so that it is similar to a 2D mammogram.
If you are interested in 3D mammography, you should check with your provider as well as your insurer regarding coverage. While 3D imaging is growing in popularity, it may not be covered in your state.

It Is Painless And Radiation
As you may have experienced or heard from other women, the pressure of a mammogram machine is equivalent to putting a 50-pound weight to compress your breasts. Many who have done mammogram will be able to tell you that it is painful. And this is especially traumatic for women who have small breasts.
It doesnt make sense to use high exposure to radiation from mammography to find what we know is caused by radiation!
With thermography, you will be required to sit in front of the scanner that scans your body heat. Thermography is non-contact, non-invasive, uses no painful breast compression, nor emits any harmful radiation.
Breast Implants May Make Cancer Harder To Detect
Breast implants may make it more difficult to detect breast cancer at an early stage, a new analysis suggests.
In the study of women with breast cancer, those with breast implants tended to be diagnosed at a later stage than women without implants.
However, the findings are preliminary, and more research is needed to confirm them.
Breast implants themselves are not thought to increase the risk of breast cancer, but there has been concern that implants may impair doctors’ ability to view breast tissue with mammograms. Earlier studies looking at this issue have had mixed results.
The new study analyzed information from 12 previous studies involving women with breast cancer who either did have or did not have breast implants. Together, these studies included women living in the United States, Canada and Europe.
Women with breast implants were 26 percent more likely to be diagnosed with breast cancer at a late stage, compared to women without breast implants. However, this finding did not quite rise to the level of “significant,” meaning it could have been due to chance.
In a separate analysis of an additional five studies of women with breast cancer, women with implants were 38 percent more likely to die of the disease than women without implants.
This second finding should also be interpreted with caution, the researchers said, because some of the studies didn’t take into account factors that could influence breast cancer risk, such as obesity.

Screening For Breast Implant Rupture
The FDA recommends that people with silicone breast implants receive MRI screening for silent rupture 3 years after the initial implant surgery and every 2 years after that. Getting health insurance to cover this can be challenging, so you may have to work with your doctors office on this.
If at any time you think your implant may have ruptured, have your plastic surgeon check it. An imaging test, such as an ultrasound or MRI, can help determine whether an implant is leaking. An MRI is widely considered the most accurate imaging test for this purpose. However, as with any imaging test, theres some risk that the results will not be accurate a rupture may not show up or an intact implant may appear to be ruptured on an MRI.

A mammogram is a type of x-ray. Breast tissue will look grey on the mammogram, and cancer will look white. In a mammogram, implants are also white and will look like a bright white orb. If there is cancer in breast tissue that is below or very near the implant on the mammogram, it probably will not be visible, and therefore wont be detected.

How Do Mammograms With Breast Implants Work
Generally, a breast screening mammogram with implants isnt that different than one without implants. There are four considerations:
Telling your health professionals upfront that you have breast implants is crucial for several reasons. Aside from the fact that the technologist will have to shift the implant to expose the muscle behind it, they will also need to use less pressure on the breast to avoid a rupture. While making your mammogram appointment, be sure to state that you have breast implants. It also wont hurt to remind the technologist of this fact once youre at the facility.
But telling them is just the first step. Its crucial to find an imaging clinic with technologists who have experience with mammograms with implants. As we mentioned, not all technologists know how to properly do breast screenings for people with implants, so ask about a clinics technologists and how often they screen people with implants. The provider needs to know precisely how to move the implant to allow them to see the breast tissue. They also must place less stress on the breasts during the exam.

What Is The Best Breast Cancer Screening For People With Implants
Because breast implants make mammograms more difficult, it is better to get a 3D mammogram than the traditional 2D screening for breast cancer. 3D images allow your doctor to see your breast tissue better, whether you have implants or not. 3D mammograms also give doctors more pictures to look through, increasing the probability that any tumors present will be seen.
Ultrasounds and MRIs can also be used in screening for breast cancer. These are other good alternatives to 2D mammograms for people with breast implants. Talk to your doctor about which type of screening is best for you.
How Is A Mammogram Different If I Have Breast Implants
For a woman with breast implants, a standard 2D mammogram includes four extra pictures , called implant displacement views.
For these views, the technologist gently pushes the implant back toward the chest wall and brings the breast tissue forward and in front of the implant, Dr. Reddy said. This allows more of the breast tissue to be evaluated.

Becoming Familiar With Augmented Breasts
Although the breast self-exam procedure following breast augmentation is similar to the procedure you followed prior to augmentation, you may need to take extra steps to familiarize yourself with your breasts and implants. First, ask your cosmetic surgeon to help you identify the edges of your implants so you can better distinguish the implants from your natural breast tissue. After youve identified where your implants reside, press firmly along the edges of the implants so you can feel your rib cage. Then, move your fingers around the edges of your implants to feel for lumps and other changes that may have developed in your natural breast tissue.
Questions To Ask About Breast Implants And Mammograms
If youre like many women, your decision to get implants came long before your need for a mammogram. The American Society of Plastic Surgeons estimated that half of the women who get breast implants are in their 20s or early 30s. The topic of screening mammograms may never have been discussed with your surgeon back then. But if you are nearing or in your 40s there are questions you should ask because women with breast implants do face different risks.
